Remediation Loop

Remediation, or the detection-to-correction loop, closes the gap between knowing an agent is failing and fixing it. The book's pipeline: catalog every agent, monitor every conversation, run continuous evals against each system prompt, cluster failures rather than chasing single cases, experiment with prompt, model, retrieval, or tool-description changes, backtest each candidate fix against historical traffic, and package validated fixes as pull requests for human review.
